How to report a claim

All claims should be reported as soon as practicable to Blue Oak Specialty Insurance Services, Inc. Claims may be reported by any of the options listed in the following table. Click for claims reporting forms.

It is important to include the policy number, the name of the insured and to provide information about the loss and those involved. An adjuster will be assigned and after reviewing the information provided will make contact the person designated by the insured to handle the claim. Please be sure to provide the full name, address, email address and phone number(s) of the individual assigned to communicate with the insurance company. A claim acknowledgement will be forwarded identifying the claim number and assigned claims examiner.
If you do not receive a claim acknowledgement within 3 to 5 days, please call 209-473-8938 and confirm that the claim information was received.
